Supporting Transformations in Long-Term Care Homes: A Logic Model to Support Individual LTC Homes in their Transformation Journeys

Any LTC home engaging in a transformation journey or specific initiative related to being more person-centred can use this logic model to support their evaluation and assist the Community of Practice Network in identifying a small number of outputs that could be tracked over time within the region.
