A Champlain Strategy: People with Young Onset Dementia and their Families

A partnership between the Champlain Dementia Network (CDN) and the Champlain Community Support Network (CCSN) was formed to develop a multi-year strategy with recommendations aimed at improving services, programs, and systems available for people living with young onset dementia in the Champlain region.
